This is an example template that I created a couple of years ago as I felt that the general school template report just didn’t fit the needs or the teaching ares of my class. I have created it in the style of the general mainstream reports as I know parents appreciate a similarity between reports from their children’s teachers in mainstream and ASD classes.

It is fully editable as a word document.
